Job Summary and Qualifications

POSITION SUMMARY:

The PPS Coordinator/Case Manager is responsible for the gathering of pertinent data, reporting this data on appropriate government forms, transmitting the data as required to Medicare, and coordinating the plan of care with the physician and treatment team. He/She will assist assuring appropriateness and cost effectiveness of treatment. This position also serves as a liaison between medical staff, administration, utilization review, nursing, finance, medical records and other ancillary departments to ensure that patient care needs are met and treatment plans are executed as well as ensuring all discharge needs are met prior to discharge.

What qualifications you will need:

REQUIRED LICENSURE/CERTIFICATIONS

  • Current RN licensure in Texas or compact state
  • BCLS certification

REQUIRED EDUCATION/EXPERIENCE

  • Minimum College: Associate Degree, Diploma in Nursing, or BSN
  • Worked as a nurse in an inpatient rehabilitation facility as part of previous employment

PREFERRED EDUCATION, LICENSURE, CERTIFICATION, EXPERIENCE, SKILLS, KNOWLEDGE AND ABILITIES

  • Attainment of CRRN or within two years working on the Inpatient Rehabilitation Unit
  • Must be proficient with the FIM instrument to assess patient functional level
  • Bilingual in English/Spanish preferred.
  • Knowledgeable of the CMS Final Rule for the Prospective Payment System governing Medicare sponsored patients in the Inpatient Rehabilitation Facility.
