HCA Florida Lake Monroe Hospital, a 221-bed acute care hospital and Level II Trauma Center, serves the communities of Seminole and West Volusia counties. The hospital provides the only full-service cardiovascular program in Seminole and West Volusia, including open heart surgery, interventional cardiology, electrophysiology, cardiac rehabilitation and comprehensive diagnostic services. Other specialized services include orthopedic and spine care, robotic surgery, acute inpatient medical rehabilitation and complete imaging services.

HCA Florida Lake Monroe Hospital is located 3.6 miles east of I-4 between S.R. 46 and Highway 17-92 in Sanford. The hospital provides 24/7 emergency care at its main location as well as at Heathrow ER located at 4525 International Parkway in Sanford. The Care Assure Nurse Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes.  S/he facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process.  The incumbent guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.  Serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.

Qualifications: 
  • Nursing Diploma, ASN or BSN; Masters preferred.
  • Previous experience in an acute care setting of 3 or more years. Specialty experience strongly preferred. 
  • Current Florida State Registered Nurse license. Current FL RN license required or appropriate compact licensure. If compact license held, active FL RN license required within 90 days of hire.
  • Current American Heart Association BLS required to start.
  • Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC Microsoft Office skills, Meditech.
