HCA Florida Putnam Hospital, a 99-bed, acute care facility, located at 611 Zeagler Dr, in Palatka, provides the latest technology with the assurance of highly trained, caring professionals to the residents of Putnam County. The hospital serves North Central Florida with a full complement of quality healthcare services while maintaining cost-effectiveness.
The hospital provides individualized care directed to the specific needs of each patient. HCA Florida Putnam Hospital offers a 10-bed Intensive/Critical Care Unit, Step-Down Care Unit, Cardiac Cath Lab, medical and surgical units, family birthplace center, and a 24-hour emergency department. We also provide inpatient and outpatient rehabilitation services.
The Physical Therapist (PT) provides assessment, treatment and rehabilitative services to patients to improve mobility, relieve pain and disability.
Designs, implements and documents appropriate initial plan of care and sets measurable goals based on patients' age, total needs perspective and environment
Demonstrates and articulates sound clinical reasoning through synthesizing evaluation data from initial patient assessment, medical history and physician orders to develop a comprehensive plan of care. Follows intervention protocols and adapts conventional therapy techniques to meet the needs of the patient. Performs regular re-evaluations to determine patient progress and adjusts or alters plan of care accordingly.
Records, monitors, and analyzes clinical data to review, reevaluate and modify treatment plan and goals.
