Social Worker - Outpatient (PHP/IOP)
SCHEDULE: PRN / Per Diem / As Needed
POSITION SUMMARY:
Supports and coordinates the social service function for patients and their families, providing discharge planning pertaining to illness, hospitalization and financial concerns and facilitates post discharge planning. Works in collaboration with the Program Director and Outpatient Coordinator to identify potential barriers to patient throughput, and quality outcomes. Works within the structure to negotiate and resolve problems and conflicts during the patient stay to ensure continuity of care.
QUALIFICATIONS:
Current BLS required
Current CPI (Crisis Prevention Intervention) required within 45 days of hire.
SKILLS, KNOWLEDGE, AND ABILITIES:
Proven ability to work with others, at all levels within the organization and collaborate effectively. Possess interpersonal problem solving skills and written/oral communication skills. Computer literacy. Ability to complete assessments within the behavioral health field such as but not limited to: psychosocial assessments, discharge assessments and intake and admission assessments.
