• Your role applies scientific principles of conditioning and motivational techniques to design exercise programs for persons with cardiopulmonary and metabolic diseases. 
• You will perform outpatient cardiopulmonary rehabilitation services in a basic case management role. 
• You will also assist with Phase One inpatient education and monitoring of cardiac core measures as necessary.  
• Your role will have delivery of care is based on criteria that is age appropriate and is based on the physical, psychosocial, cultural, safety, and learning needs of the individual patients.  
• You will participate in Quality Assurance and Performance activities for the department.
