Job Description:
The CM Assistant under the direction and supervision of the Director of Case Management is responsible for providing the following processes: distribution and documentation for the following: Important Message from MCR, Observation pamphlets, Medicare Waiver Transport Letter, Patient Choice Letters, Bookings of discharged patients to alternate levels of care such as: SNF, Rehab & LTAC. Electronically faxing all pertinent information via MIDAS, to KEPRO for all Medicare appeals. This individual will be assigned to multiple units. The CM Assistant may also become responsible for additional clerical responsibilities as the Director identifies them.
  • Greets visitors and directs visitors appropriately, maintains communication flow.
  • Able to perform designated daily activities with little or no supervision.
  • Performs specific audits as requested by Director of Case Management.
  • Demonstrates support for change within the Case Management department.
  • Demonstrate excellent verbal and communication skills with CM team, patients and families/guardians.
  • Distribute, document and run Midas audits for compliance daily on the following items: Important Message from Medicare, Medicare Transportation Waiver, Observation Pamphlet, documents for appeal in Midas, bookings for patients discharged to: SNFs, Rehabs and LTACs, assist with signing of Patient Choice Letters.
  • Utilizes MIDAS for Case Management processes.
  • Follows through with assignments and informs supervisor if unable to complete task(s).


  • High School Diploma required
  • Able to effectively communicate in English, both verbally and in writing.
  • Excellent computer skills
  • Ability to accept change readily.
  • Ability to effectively communicate
