Position Summary & Responsibilities:
The Registered Nurse Case Manager Inpatient Rehab Under general supervision of the Program Director of Inpatient Rehabilitation, the Inpatient Rehabilitation Unit Case Manager is responsible for leading care coordination and assessment of discharge plan for patients requiring inpatient rehabilitation. Concurrently screens patients to identify and facilitate discharge planning needs. Coordinates discharge plans with individual patients, families, physicians and other members of the health team. Functions in a case management role for the Inpatient Rehab Unit, coordinating the care and service of the rehab population across the continuum, promote effective utilization, monitor health care resources, and assume a leadership role with the interdisciplinary team to achieve optimal outcomes using the tools of case management.is part of a collaborative process that assesses plans, implements, coordinates, monitors and evaluations health care delivery systems. Based on the needs and values of patients and in collaboration with all service providers, the case manager links patients with appropriate resources throughout the continuum of health care settings to assist with achievement of optimal health.

Minimum Position Requirements:
a. TX RN License or compact license
b. Certification in Case Management (CCM) (desired)
a. ADN required.
b. BSN preferred. If hired into RN required position, enroll in a BSN program within 6 months of hire and obtain in 36 months
a. Minimal 5 years of managerial or supervisory case management experience
b. Knowledge of Milliman's Care Guideline, Interqual, and regulatory agencies
c. At least 2 years recent or past medical/surgical/ED/ICU/PCU experience
d. Must maintain current acute care clinical knowledge
e. Ability to assess discharge planning needs of a diverse patient population
