Behavioral Health Counselor / Social Worker
Medical City Lewisville
Position Summary:
To provide behavioral health counselling services and intervention to patients (adult/geriatric) and patient families. To serve as a resource person to internal staff in psychosocial issues. To maintain accurate and current records on all patients according to the department’s policies and procedures manual and regulatory standards of practice.
Job Duties and Responsibilities:

Participates in hospital or departmental performance improvement activities.

Assists physician, ASSOCIATES, patient families and patient with discharge planning.

Evaluates patient and family responses to discharge planning intervention.
-
Serves as information and referral resource regarding community services.
-
Counsels patients and patient families.
-
Maintains and updates the social service policy and procedure manual, as indicated.

Performs all appropriate patient documentation.
-
Follows age specific discharge planning criteria.

Serves as a resource to department staff, students, volunteers and other departments, and contributes to the teamwork essential to ensuring quality patient care

Takes appropriate measures to ensure safety of patients.

Participates in staff meetings, patient care conferences and other committees as required

Participates in the orientation of new employees and cross training of other social workers as indicated

Participates in mandatory department and hospital in-services and education programs
-
Maintains on-going education to improve job performance

Demonstrates sensitivities to culturally diverse patient population

Demonstrates understanding of HCA, Medical Center of Lewisville mission, guiding principles and incorporates them into daily performance

Proactively seeks team building and positive working relationships with all hospital and medical staff

Participates in hospital or departmental performance improvement activities, and seeks opportunities to improve department and inter-department processes

Supports and participates in hospital safety program, infection control and other processes to ensure a safe environment

Demonstrates flexibility regarding assignments

Perform special assignments as needed/requested
Qualifications
License/Certification:
Education:
-
Accredited by the council on Clinical Social Work Education.
-
Completion of Clinical Social Work curriculum
-
Master’s degree in Social Work preferred

Good interpersonal, and verbal & written communication skills.

Able to work independently and prioritize responsibilities.

Knowledgeable of psychosocial and physical needs of adult and geriatric patients.
