Job Summary and Qualifications

The Care Assure Nurse Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes and will facilitate patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. Our team is amazing, we consider ourselves a family who work hard to support each other and we are looking for someone who feels patient care is as meaningful as we do. In this role:

·You will receive and review referrals for appropriateness, reviewing with an attending as needed.

·You will be a resource to both the patient and family explaining steps in the process and work-up.

·You will obtain all demonstrate awareness and respect to the patient, family and/or significant others as an individual and family unit.

·You will connect patient to community and social support services and serve as a key contact for community awareness.

·You will educate the patient regarding planned procedures, medications, treatments and is able to develop a patient specific teaching.

·You will serve as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.

·You will evaluate clinical educational needs of staff related to target population and facilitates collaboration with clinical care givers.


·Registered Nurse, Current Utah State Nursing License.

·Bachelors or Master’s degree in nursing preferred

    ·Minimum 3 - 5 years relevant specialty area experience (i.e. Cardiovascular, Orthopedic, Neuro, Spine, and Oncology specialty area experience strongly preferred. Knowledge and application of pertinent computer programs including Microsoft office, and imaging software (I.E. MUSE, PACS, etc.)
