The medical social worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.

What you will do in this role:

  • Acts as a liaison between the patient and community resources/post-acute care providers.
  • Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care.
  • Facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput. In addition.
  • Provides orientation and mentorship to new and existing staff.
  • Collaborates with the Director of Case Management in the hiring and interview process for new professional staff and manages a limited case load in addition to providing guidance and consultation to social work staff.
  • Excellent analytical and team building skills as well as the ability to prioritize and work independently is required.

Qualifications

  • Master of Social Work (MSW) required
  • State licensure required (may be required by state regulations)
  • Certification in social work or case management preferred
  • Five years acute care social work experience required
  • Knowledgeable about internal, community and post-acute care resources
  • Basic social worker skills including but not limited to empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention
