Position Summary:  The Social Worker (MSW) I reports to the Manager of Case Management and Care Transitions. Under guidance of Lead Social Worker, the Social Worker I (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.  

 

 

 

 

 

 

 

 

 

 

 

 

 

 

Qualifications

Licensure/Certification/Registration: MSW licensure by the South Carolina Board of Social Work Examiners. 

                   

Education:

Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. MSW licensure by the South Carolina Board of Social Work Examiners.  Certification in case management preferred.  Requires excellent communication skills, attention to detail, and ability to work independently.  Demonstrates ability to collaborate in a positive, professional manner with medical professionals.  Requires ability to work in a thorough but efficient manner.

  Experience: 

Performs in accordance with delineated job responsibilities and satisfactorily completes a yearly evaluation based on these responsibilities.

 

Must possess the knowledge and skills necessary to effectively meet the needs of the patient/internal & external customer.

 

 

Special qualifications: 

Ability to establish and maintain collaborative and effective working relationships.

Ability to communicate effectively in oral, written and electronic formats.

Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills.

Demonstrates an ability to perform specific competencies as identified on the Case Management Competency Grid.

 

Degree of Supervision Required:

 

Involves general guidance and direction by Department director.  The employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.
