We are seeking a Nurse Navigator to ensure that we continue to provide all patients with high quality, efficient care. Here is what you can expect from working as a Nurse Navigator for Sky Ridge Medical Center:

  • You will serve as patient advocate throughout continuum of care.
  • You will assess patients’ medical, social and psychosocial and other care needs.
  • You will provide appropriate teaching, outreach, and education to patients and families. The aim of this work being to ensure the patient is empowered to manage his or her own health.
  • You will streamline care coordination and paperwork by helping patients with scheduling appointments and preparation.
  • You will coach and help patients to remove barriers with issues of insurance, transportation, child care, financial resources, language so they may focus on getting the care they need.
  • You will initiate referrals to hospital and community resources to connect patients with resources and support systems.
  • You will conduct follow-up conversations as needed with all patients and communicate concerns, changes, or social needs in patient health to appropriate MD or other appropriate care providers.
  • You will attend patient care planning conference and other meetings as necessary.
  • You will track and document interventions and outcomes.
  • You will drive process improvement.
  • You will stay current on the latest service line developments/education and participate in conferences.
  • You will ensure service continuity.
  • You will conduct outreach to referrers, providers, other medical professionals and other key partnering organizations
  • You will undertake other duties as assigned.

 

Requirements for a Nurse Navigator:

  • Bachelors degree required
  • 1-3 years overall nursing experience
  • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license
