Medical City Denton is a 208 bed, full service hospital that serves the growing population of Denton, Wise, Cooke and Montague Counties. Heart Failure Coordinator performs direct patient care within the scope of the nurse pactice act, as it applied to the outcomes and processes in teh Advanced Heart Failure Clinic. Plan, coordinate and implament the care of the Heart Failure patient in the outpatient setting. Clinic referral management to include interaction with office staff and providers, HIM needs. Enact all orders and pateint care needs as identified by the HF APP team and Physicians. Scope of the role inc;udes acting as a liaison with the physicians, patient/families and ancillary departments. Assessment of finacial, medical and social resources of current and potential HF program patients. Coordination of inforamation flow among the medical , nursing, financial, and research staff as appropriate. Develop and implament a teaching plan with the patient and family. To include lab values, medication overview and diagnosis related information. Other duties as assigned. Assist the research team in the coordination of patient charts, diagnostic studies and other appplicable information per research protocol.

Qualifications

  • ADN required, BSN enrollment within 6 months of hire and completed within 24  months.
  • Two years cardiology expereince preferred
  • Current RN license in the state of Texas or compact license
  • BLS from AHA or ARC
