Spotsylvania Regional Medical Center is a 133-bed facility that opened June 7, 2010.  SRMC provides a wide range of inpatient and outpatient services including emergency, women’s services, orthopedic, cardiac care and behavioral health services.  Our surgical services include robotic-assisted technology, allowing our surgeons to operate with the tiniest incisions and with greater vision, precision and control.

What will you do?
  • Exhibits excellent customer relations to patients, visitors, physicians, and co-workers. Shows courtesy, compassion and respect to all individuals.

  • Utilize team work to complete daily tasks.

  • Screen admissions for discharge needs within one-two (1-2) business days.

  • Track and monitor avoidable (alternative care) days, and provide your supervisor with this information daily.

  • Performs continuous assessments and evaluations, to ensure patient is progressing towards desired outcomes.

  • Assesses and responds to patient/family needs by coordinating efforts of other team members.
  • Identifies and resolves barriers that hinder effective patient care.
  • Collaborates and consults with physicians on patient's progress and discharge planning needs.
  • Communicates effective and professionally with peers, supervisors, ancillary departments and all other external/internal customers.
  • Manages the process to review and, as appropriate, appeal denials received from payors.
  • Coordinates the discharge planning process ensuring involvement of all members of the healthcare team.  Counsels with patients and family members in decision making and in meeting psychosocial needs of the patient.
  • Maintains knowledge of resources and facilities available to patients and family members. 
  • Collaborates with personnel at other facilities to coordinate smooth and effective patient transfers and transitions.
  • Compiles, evaluates, and reports statistics to members of the team and utilizes the information to facilitate process improvement activities.
  • Analyzes pertinent data, communicates findings, documents information accurately.
  • Provides age and culturally appropriate care.
  • Orients and mentors new staff members.
  • Follows Standard Precautions using personal protective equipment as required.
  • Acquires/maintains current knowledge related to Case Management.
  • Utilizes patient choice letters as appropriate.

  • Provides “Important Message from Medicare About your Rights” letter within appropriate timelines.

  • Completes UAI paperwork as necessary.

  • Assists with the process of adoption when the need arises.

  • Collaborates with RN Case Manager to coordinate smooth and effective patient transfers and transitions.

  • Customer service abilities including effective listening skills.

  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision meeting firm deadlines.

  • Assertive and persuasive communication skills, action oriented.

  • Minimum of 5 years of social work/case management experience required. 


  • Bachelor of Science in Social Work required

  • Master of Social Work required.


  • Current state licensure as LCSW preferred.

  • Certified Case Manager preferred.

  • Supervision Hours available with employment duration committment.
