Registered Nurse Clinical Nurse Coordinator

Shift: Full-Time Days w/ Rotating Weekends

 

The Clinical Nurse Coordinator (CNC) Registered Nurse ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization’s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.
• Collaborates with a multidisciplinary team to coordinate, evaluate, customize, and deliver high quality, patient-centered care
• Supervises assigned personnel assuring quality patient care and efficient and effective work processes
• Supports the achievement of internal/external benchmarks, accreditations, and designations
• Assists with staff scheduling and manages all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines
• Supports the efforts of the facility to improve engagement by operationalizing current nursing strategies, including employee rounding, hourly rounds, and other initiatives

The Progressive Care Unit / ICU Stepdown RN  provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.  The tasks and responsibilities include:
  • Assesses plans and evaluates patient care needs. 
  • Carries out physician orders.
  • Administers prescribed medications, changes dressings, cleans wounds, monitors vital signs.
  • Serves as the primary coordinator of all disciplines for well coordinated patient care.
  • Monitors, records and communicates patient condition as appropriate utilizing computerized documentation systems. 
  • Instructs and educates patients and families.
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
  • Provides age and culturally appropriate care.
  • Orients and mentors new staff members.
  • Follows Standard Precautions using personal protective equipment as required.

Qualifications

  • Position RequirementsLicensure / Certification / Registration:

  • Education
      • Graduate of an accredited school of nursing.
      • BSN required within 1 year. Magnet®/Pathways® Accredited hospitals require BSN at time of hire.
      • Certification in specialty required within 1 year.
      • Completion of charge nurse course required within one year.
      • Other leadership training and activity in professional organizations highly preferred.
