Clinical Social Worker - Mission Health Partners

 

Full-Time - Days

 

The Mission Health Partners (MHP) Clinical Social Worker provides clinical psychosocial assessment for patients identified as having significant emotional, social, substance abuse, or mental issues. Provides patient and family advocacy; clinical psychosocial assessment, diagnosis, and appropriate treatment and intervention. Supports MHP team as a resource to manage complex patients. Collaborates with community partners to decrease duplication and enhance community collaboration in patient care.

 

Required Education: MSW

Preferred Education: None

 

Required License: LCSWA and pass ASWB Clinical Evaluation Examination within two years of LCSWA issue date.

Preferred License: LCAS is preferred but not required.

 

Required Experience: Experience providing clinical psychosocial assessments in medical or psychiatric/substance abuse settings. (Can be a field placement.) LCSWAs must complete clinical supervision hours over a period of time no less than two years, or more than six years and obtain LCSW.

Preferred Experience:  5 years work experience providing clinical psychosocial assessments in medical or psychiatric/substance abuse settings.' Understanding and sensitivity regarding challenges facing low-income communities.' Entry-level proficiency in the Spanish language preferred. Computer proficiency electronic health record experience preferred. Ability to flourish in a team system.

The Mission Health Partners Care Coordination team – available to all MHP providers - includes registered nurse (RN) care coordinators, certified pharmacy technicians, licensed clinical social workers (LCSWs) and clinical pharmacists. The team collaborates with our physicians and regional community partners, expanding our scope and ensuring that even our most rural patients are able to receive local and specialized assistance when they need it.

 

Mission Health Partners (MHP) currently serves approximately 90,000 patients in western North Carolina, with an overall goal of improving health outcomes and reducing costs. MHP utilizes a unique and innovative combination of data-driven initiatives and the implementation of an innovative care coordination model that focuses on social determinants of health – that is, the social and environmental factors that may have a significant influence on a patient’s health outcomes.
