CarePartners Home Health provides Nursing, CNA, Therapy and Social Work services to people across the WNC region. These services make it possible to provide high quality, personal, and compassionate care for patients in their homes. Home Health staff truly get to know their patients and have shown to be satisfied with their jobs according to in house employee surveys. Home Health staff must be 21 years old, have a good driving record, reliable transportation (for which mileage is reimbursed), and must have proof of insurance with their name on the policy.
Job Summary: The Primary Occupational Therapist in Home Health manages an assigned caseload of patients, providing therapy and clinical case management, taking responsibility to improve patient outcomes and to manage resources and assure cost effective, clinically effective, and coordinated services. Completes comprehensive assessment/examination/evaluation, develops/modifies plan of care in conjunction with patient/caregiver and physician, provides treatment visits, and provides discharge planning to these patients to facilitate patient/caregiver engagement for achievement of patient goals, clinical outcomes and positive patient experience. The Primary Occupational Therapist provides the majority of patient's occupational therapy and communicates handoff information/coordinates with other therapists/members of care team to support patient's care throughout home health and the continuum of care.
Required Education: Bachelor of Science degree in Occupational Therapy
Required License: Current North Carolina license to practice Occupational Therapy; Driver's license required. BLS/CPR Certification
Required Experience: One to two years related experience.
