The Transplant clinic LVN performs high quality direct patient care within the scope of the licensed Vocational Nurse Practice Act, with emphasis on the out-patient in the Transplant Center:
• Care for adult patients in the Solid Organ Transplant Clinic under supervision of RN
• Patients teaching: medication, procedures, follow up and other identified needs
• Physician clinic support
• Patient scheduling
• Gather, enter and maintain data/documentation related to Transplant Clinic patients consistent with established quality and integrity standards
• Participate in Quality Improvement
• Point of Care Testing
• Follows through on patients’ needs through coordination of services
• Supports the medical record integrity
• All other duties as assigned
