Position Summary & Responsibilities:
The Registered Nurse Case Manager is part of a collaborative process that assesses plans, implements, coordinates, monitors and evaluations health care delivery systems. Based on the needs and values of patients and in collaboration with all service providers, the case manager links patients with appropriate resources throughout the continuum of health care settings to assist with achievement of optimal health. Case management is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes.

Minimum Position Requirements:
a. TX RN License or compact license
b. Certification in Case Management (CCM) (desired)
a. ADN required.
b. BSN preferred. If hired into RN required position after September 2014 and no
BSN, then BSN required within three years of facility date.
a. Minimal 5 years of managerial or supervisory case management experience
b. Knowledge of Milliman's Care Guideline, Interqual, and regulatory agencies
c. At least 2 years recent or past medical/surgical/ED/ICU/PCU experience
d. Must maintain current acute care clinical knowledge
e. Ability to assess discharge planning needs of a diverse patient population
