Position Summary
Responsible for professional nursing care and related assistance to patients with a broad range of illnesses. The RN is involved in direct and indirect patient care. The RN provides direction to licensed vocational nurses (LVNs) and other non-licensed nursing personnel in providing direct patient care. The staff nurse supports the medical care of patients under the direction of the Medical Staff pursuant to the objectives of the hospital. In collaboration with other health team members, follows the philosophy, policies and goals of the facility and department.
Job Function

Performs a complete and accurate nursing assessment on assigned patients.

  • Completes admission interview accurately within 24 hours of patient admission.

  • Performs and legibly documents patient assessment completely and accurately per shift or more frequently, as indicated by patient condition.

  • Identifies changes in patient condition (i.e. test results, abnormal findings, etc.) and reports to the physician and/or Supervisor, Nursing Unit.

  • Assesses and documents patient’s educational needs on admission and throughout hospitalization.


Plans comprehensive nursing care for assigned patients according to written hospital standards and physician orders.

  • Initiates and individualizes appropriate areas of the MITP within 24 hours of admission.

  • Identifies and prioritizes relevant nursing diagnoses.

  • Delegates appropriate nursing duties to PCAs and PCTs

  • Reviews and updates MITP daily.

  • Initiates discharge planning upon admission.


Provides and legibly documents skilled care to assigned patients in compliance with written hospital policies and procedures and according to physician’s orders.

  • Performs and legibly documents nursing care according to patient needs in a competent and timely manner.  Documents FIM scores accurately for the first 72 hours of a patient stay in Rehab and on the MITP.

  • Accurately administers and legibly documents medications within 30 minutes of indicated time or as patient condition warrants.

  • Provides individualized nursing interventions based on identified nursing diagnoses and plan of care.

  • Initiates appropriate teaching and/or referrals.

  • Carries out appropriate interventions based on identified changes in patient condition.


Evaluates patient care outcomes in an appropriate and timely manner.

  • Evaluates nursing plan of care on the MITP to determine need for modifications on a daily basis.

  • Documents resolution of patient problems identified in plan of care.

  • Evaluates effectiveness of patient teaching and revises teaching plan as indicated.

  • Participates in Quality Improvement activities.


Maintains current professional nursing knowledge.

  • Documents evidence of a minimum of 8 hours of in-house staff development, in addition to mandatory education requirements, or continuing education relevant to the clinical practice area.

  • Completes unit specific competency checklist and age specific competency requirements.

  • Obtains and maintains FIM certification.



Education and Experience
Previous experience in clinical nursing
Prior inpatient rehab nursing experience preferred
Computer skills 

Certifications and Licenses

Current licensure in Texas as a RN
Certification in basic cardiopulmonary resuscitation.
