The Social Worker has the opportunity to assess the patient’s psychosocial status, to confirm probably post-acute needs, and to detect any potential barriers to a successful transition in to the community. Social Worker
collaborates with the case manager, the physician and family to determine the best course of action. The final discharge plan is left to the prudent collaboration of the case manager and the social worker.
1. Screen all patients on assigned unit/service line by covering the selected number of key screening indicators that consistently signal the need for a more in-depth transition plan evaluation by the social worker.
2. Respond to all consults in a timely and efficient manner.
3. Collaboration with case manager, nursing staff, ancillary departments and patient/family in formulation of the transition plan.
