The Social Worker is one of the mental health providers on the Prenatal Plus team at Rose Medical Center. The purpose of the Prenatal Plus team is to improve the maternal and infant health outcomes of at-risk Medicaid clients be providing comprehensive and coordinated prenatal and early postpartum support services that complement traditional clinical prenatal care. The primary goal of the program is to reduce the incidence of low birth weight babies while also addressing other lifestyle, behavioral, and non-medical aspects of a woman’s life that may affect her and/or her baby’s health and well-being. These goals are achieved by focusing on case management, psychosocial counseling and support, client psychoeducation and health promotion, and nutrition counseling and support. The Prenatal Plus team offers support services in an integrated care model to ensure that women have access to the services and information needed to have healthy pregnancies and healthy babies.
The Social Worker duties include:
- Performs comprehensive psychosocial health assessments and screenings.
- Delivers psychoeducation about perinatal mood and anxiety disorders in a group or individual setting.
- Provides psychosocial health counseling and support, including psychosocial care-planning, goal-setting, monitoring, follow-up, and psychosocial care plan revision.
- Coordinates and communicates pertinent client information with Prenatal Plus team of providers and with the client’s OB provider, if appropriate for client’s care.
- Maintains up-to-date clinical notes at all times.
- Utilizes proper referral mechanisms and actively seeks out new referrals for clients when appropriate.
- Helps deliver community referrals, general education and health promotion around a variety of topics relevant to a healthy pregnancy.
- Co-leads a weekly postpartum support group for new mothers.
- Potentially provide clinical supervision of unlicensed clinician(s).
- Performs delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of care at RMC
- Displays positive support for the values and mission of RMC.
Current Colorado LCSW, LPC, or equivalent licensure required.
Master’s level professional in the field of social work, professional counseling, marriage and family therapy, or equivalent behavioral health specialty.
3-5 years recent clinical work experience, experience in perinatal field or clientele preferred.
Must have the ability to assess information, apply critical thinking processes to determine course of action, and relay this information to the interdisciplinary team.
Requires excellent human relations and oral/written communication skills. Must be able to prioritize workload, and develop efficient methods of managing time.
