  1. Serve as the liaison between the patient and Director to coordinate discharge care.
  1. Demonstrate excellent oral and communication skills and customer service skills.
  1. Must demonstrate leadership role modeling and problem resolution skills. 
  1. Knowledgeable of computer and phone applications.
  1. Must successfully complete orientation and maintain requirements.
  1. Maintain greater than 75th percentile for discharge phone calls on all inpatients discharged and surveyed through Press Ganey.
  1. Provide Directors, CNO, ACNO, and Director of Clinical Operations daily updates on discharge phone calls.
  1. Assist in identifying trends, recognition, delays in care, or discharge disposition issues.
  1. Assist in facilitating activities that will promote the patient experience.


Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation.

Basic Cardiac Life Support

Nursing graduate with RN Diploma or Associate of Science in Nursing

Minimum of 2 years acute care nursing.

Bachelor of Science in Nursing
