Physician Assistant Cardiology Clinics - Part Time
The Physician Assistant working within the Cardiology Clinics demonstrates critical thinking skills, scientific judgment and leadership in planning for the care of the patient. The Physician Assistant is responsible and accountable for the planning, directing and provision of care to patients in accordance with the NH Board of Medicine, recognized through clinical and administrative policies. The Physician Assistant working in the Cardiology Clinics will be required to participate in CQI initiatives.
SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS:
Demonstrates sensitivity and respect to the family and/or significant other as an individual and family unit.
Identifies cultural and religious needs, language barriers, developmental needs, etc., and modifies plan of care as necessary to accommodate those needs.
Provides patient education at the survival skills level (self care behaviors) in the areas of HF to prevent further complications.
Develops and implements processes for identifying HF.
Functions as a clinical liaison to link inpatient to outpatient care.
Demonstrates initiative and actively participation in disease-specific committees, e.g. heart failure.
Anticipates changes in acuity, needs/problems and expectations through continuing evaluation of patient data; implements care as appropriate.
Participates in daily patient care rounds or report and identifies appropriate concerns and needs for the patient and family.
Facilitates/schedules appropriate follow-up for medical and educational needs within 1-2 weeks from discharge.
Participates in Performance Improvement activities to improve patient care and improve support services for patients and their families.
Maintain established hospital and departmental policies and procedures, objectives, confidentiality, quality improvement program, compliance, safety, infection control and environmental standards.
Maintain professional growth and development through attendance at seminars, workshops, conferences on in-services, professional affiliations or journal to stay abreast of current trends in cardiovascular care (i.e. pacemaker/ICD management).
Develops a relationship with discharging providers, case management, nursing and other professionals and makes recommendations for care consistent with science based consensus guidelines.
Works within the diagnosis and assessment of cardiology particularly those with heart failure. Evaluation will include heart failure staging in accordance with NYHA and AHA guidelines.
Acts as a resource in regards to patient and staff cardiac educational needs.
Meets regularly with Medical Director to review anticoagulation program/organizational policy needs and changes at a minimum quarterly.
Make dose management decisions based on clinic protocols and INR results

Post Graduate Degree
3-7 Years of relevant experience as a PA
