We are searching for an Area Director of PAC (Post-Acute Care) Network Partnerships for Rio Grande, Valley Regional, and Rapides Markets to join our team. You will develop and maintain an ongoing collaborative relationship with PAC providers based on mutual respect, high quality outcomes and patient satisfaction.  You will also work to continually shape the Network landscape by offering objective data and education to all stakeholders in the market.   You will play a vital role in achieving positive patient outcomes in the market including but not limited to: improving Skilled Nursing Facility (SNF) length of stay (LOS), increasing the efficiency of PAC provider’s delivery of care, increasing discharges to Preferred PAC Providers, monitoring of readmission trends and tracking quality of care measures across all Preferred PAC Providers, and educating hospital and PAC leaders on strategies to improve performance.

Duties and Responsibilities: 
  • Responsible for developing a network of preferred post-acute care (PAC) providers for a defined list of hospitals. 
  • Responsible for participation in the hiring, training and supervision of other clinical or operational field staff as appropriate.  
  • Ensures all necessary logistics obtained to support a successful Preferred PAC network (contacts, EMR access, onsite access, etc.).
  • Responsible for maintaining positive relationships with PAC providers and hospital staff to support the patient-centric model in a highly collaborative PAC network.
  • Provides regular onsite reviews of PAC provider operations to ensure quality patient care and staff support and communications results to hospital and PAC leadership. 
  • Executes department goals by leading initiatives, meeting deadlines, and coordinating resources between hospitals and Preferred PAC Providers to ensure efficient and effective operations.
  • Gathers and monitors relevant data regarding the PAC provider community and the changing dynamics within the market. 
  • Monitors reporting database of PAC provider metrics (e.g. readmission rates, ALOS, publicly reported quality metrics, patient experience, etc.).
  • Identifies challenging situations early and ensures appropriate action taken to mitigate escalation, works to resolve PAC provider complaints, and leads PAC network issue resolution.


What you should have: 

  • A degree in therapy, nursing, social work, or healthcare related field is preferred
  • 3+ years of working experience within a healthcare setting
  • Experience in Acute and Post-Acute Care Continuum required
  • Demonstrated ability to manage remote, direct reports and/or team members 
  • Demonstrated ability to independently plan, organize and prioritize work
  • Executive communication and presentation skills required
  • Effective negotiation and influencing skills
  • Ability to build and foster relationships inside and outside the organization
  • Demonstrated willingness to work toward team objectives
  • Possess a valid driver's license
  • Data analytics and reporting experience preferred
  • Experience in Value Based Care models, managed care, customer service, and provider relations preferred
  • Proficiency with Microsoft Office applications, specifically Excel and PowerPoint preferred
