POSITION SUMMARY: The Registered Nurse demonstrates critical thinking skills, scientific judgment and leadership in planning for the care of the patient. The Registered Nurse is responsible and accountable for planning, directing and provision of nursing care to patients in accordance with the NH Nurse Practice Act, recognized nursing techniques, standards of care for Palliative care, and both clinical and administrative responsibilities.

 

 

SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS:

  • Palliative care nurse demonstrates good organizational skills, independent critical thinking, patient advocacy, nursing assessments and technical skills required to care for the palliative care patient population.
  • Knowledge of palliative care nursing and population specific needs including and not limited to: consultation and assessment, end-of-life discussions, plan and goals of care, and support, family and caregiver education and support, bereavement issues, spiritual support, standards of care, discharge planning.
  • The Palliative care nurse works collaboratively with the palliative care team to assure patient dignity and patient and family satisfaction. The palliative care nurse demonstrates a holistic, caring, patient centered model of patient care.

Qualifications

Education Required and/or Preferred:

  • Diploma or Associates Degree from a professional nursing program (required)
  • BSN or current enrollment in a BSN program at an accredited school of Nursing (preferred)


Licensure/Certification:
  • Current Registered Nurse License in New Hampshire or compact state

  • BLS


Experience:
  • Acute care experience preferred

  • 1 to 3 years
