SCHEDULE: Full-time

Regional Medical Center of San Jose is an acute care hospital with a Level II Trauma Center. The hospital provides a host of technologically-advanced services including Cardiovascular, NeuroOrthopedic, Bariatric and General Surgery, Interventional Pulmonology and services for Women . Regional holds Joint Commission advanced certification as a Comprehensive Stroke Center and a Get with the Guidelines- Stroke Gold Plus Performance Achievement Award from the American Heart Association/American Stroke Association. It is a certified Chest Pain Center, county-designated STEMI (heart attack) Receiving Center, designated Lung Cancer Screening Center, a member of Extracorporeal Life Support Organization (ECMO) and is certified by The Joint Commission in Sepsis care.

Schedule: 1.0 (40 hours per week)  Shift: Standard days, no weekends
 
 
POSITION SUMMARY;

The Clinical Documentation Spec I conducts a comprehensive concurrent review of clinical documentation within the medical record to achieve accurate and detailed documentation.  Appropriate provider documentation for clinically indicated conditions or procedures supports appropriate assignment of severity of illness, expected risk of mortality, intensity of service and resource consumption, which in turn accurately portrays the facility’s quality outcomes ratings, reduces compliance risks, and captures appropriate reimbursement.


Qualifications

QUALIFICATIONS
 

 LICENSE/CERTIFICATION REQUIREMENTS:

 
 Minimum: Current RN license required with Critical Care, OR; or RHIA/RHIT/CCS certification   required; or MBBS/MD required. CCDS, CDIP required                                                           Preferred: ER Experience                                                                                                                                                                                                                                                                         EXPERIENCE REQUIREMENTS:

 Minimum:  2-5 years CDI experience in an acute care facility required


 EDUCATION REQUIREMENTS:

 Minimum: College Graduate Required Undergrad; Degree in Health Information Management,   Medicine or Nursing required

 Preferred: BS degree preferred.
