POSITION SUMMARY: 

The Social Worker uses expertise and insight to evaluate the emotional, safety, psychosocial and discharge needs of the most difficult patients and/or family members. The Social Worker will provide appropriate intervention or resources, ensuring the IDT team is meeting social and emotional needs related to the patient's hospitalization and work to provide a safe discharge plan. 

The Social Worker is instrumental in removing potential barriers to patient throughput and quality outcomes and facilitates appropriate discharge plans, collaborating with the RN case manager. They help with crisis assessment and intervention, debriefing, brief counsel and mental health therapy as defined by their clinical state licensure and participate in oversight of social work students and co-workers that require licensing supervision as requested.

Qualifications

EDUCATION REQUIREMENTS:
  • Master's of Social Work required
  • Knowledge of State of Alaska law, Federal/CMS regulations and Joint Commission standards
 LICENSURE/CERTIFICATION:
  • Alaska State LCSW or LMSW required
EXPERIENCE:
  • 1-3 years Hospital Social Work or Case Management preferred, as LCSW
