    Job Summary and Qualifications

    The Social Worker for our department will provide clinical expertise to ensure all patients receive high-quality, efficient care. Our dynamic team works hard to support each other and to ensure the delivery of excellent patient care. In this role you will have the ability to positively impact and improve lives.

    What you will do in this role:

    · You will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management.

    · You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

    · You will identify potential barriers to patient throughput and quality outcomes

    · You will evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.

    · You will act as a liaison between the facility and resources external to the organization.

    · You will help with the facilitation and implementation of timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care

    · You will provide crisis intervention and support to patient/family support systems.

    What Qualifications you will need:

    · Licensure as a LMSW is required, LCSW is highly preferred

    · Master’s degree in Social Work Required

    · 3-5 years of experience in hospital patient care preferred

    What qualifications you will need:


    • Basic Cardiac Life Support must be obtained within 30 days of employment start date
    • Nonviolent Crisis Intervention must be obtained within 90 days of employment start date
    • Licensed Masters Social Worker (LMSW)
    • Masters Degree

    This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).

    Live Oak Mental Health and Wellness Center, is a part of Trident Health. We give inpatient stabilization treatment to patients who seek treatment for acute psychiatric symptoms which cannot be addressed on an outpatient basis. Common disorders of patients treated in this program include but are not limited to: major depressive disorder, schizoaffective disorder, schizophrenia, bipolar disorder, adjustment disorders, post-traumatic stress disorder, impulse control disorder and a variety of co-morbid personality disorders. Our patient population is both adult and adolescent. We offer 24 hour availability of a full range of services. We have the capability to institute rapid lifesaving interventions. Our physician availability is 24 hours a day to institute needed modification to the treatment plan. Psychiatrist involved in the direction and management of the treatment program. Round the clock skilled nursing care implementing the treatment plan and providing monitoring and assessment of the patient’s condition and response to treatment.
