Job Summary and Qualifications

What you will do in this role:

• In collaboration with the medical and facility staff, provides and coordinates multidisciplinary care for the septic patient and the stroke patient.
• The scope of practice includes ensuring organized care is provided to the patient with a sepsis or stroke diagnosis with a focus on processes and outcomes of care, oversight of compliance with all disease management related tactics, and documentation, as well as data collection, aggregation, analysis, and presentation.
• The Sepsis/Stroke Coordinator’s role focuses on integrity of medical record documentation through appropriate interactions with physicians, nursing staff, other patient caregivers.

What is required for this role:

• Bachelor of Science in Nursing or related healthcare discipline (required)
• Master's Degree (preferred)
• Minimum of 5 years recent experience in hospital acute care setting (required)
• Knowledge of clinical documentation systems/EHR, age and culture-specific competencies (required)
• Working knowledge of reimbursement and coding structures (preferred)
• Equivalent combination of education and/or experience may be considered
• South Caroline RN License (required)
• Basic Life Support - BLS/BCLS (American Red Cross or American Heart Association) - Course/Certification (required)
• Advanced Life Support - ALS/ACLS - Course/Certification (required within 90 days of employment)
• Pediatric Advanced Life Support - PALS - Course/Certification (required within 90 days of employment)
