Seeking an experienced Behavioral Health Practitioner to join our dynamic team of mental health professionals. In this role, you will conduct ongoing assessments of the psychosocial needs of patients in collaboration with the rest of the behavioral health recover team. · You must possess an in-depth and current knowledge of community resources and post-hospitalization treatment options in order to facilitate transitions to the appropriate next level of care.
· One of your primary functions will be to collaborate with the behavioral health recovery team, patient, appropriate family members and community partners to develop and revise recovery plans and coordination of services.
· You will facilitate educational groups and family meetings to prepare for patients’ post-hospitalization treatment needs.
· You must also monitor patient acuity and progress and then coordinate discharge planning when appropriate.
· You will provide ongoing education regarding ethical, psychosocial and discharge planning for the members of the interdisciplinary treatment team.
Requirements:
· A minimum of 2-5 years clinical behavioral health experience commensurate with educational level.
· Discharge planning and individual/group/family therapy experience preferred.
· Graduation with a MA/MS in a behavioral care discipline.
· Licensed in the State of Virginia as a Professional Counselor or Licensed Clinical Social Worker or License Eligible Under Current Approved Supervision. LCSW preferred.
· American Heart Association or American Red Cross Basic Life Support (BLS) Health Care Provider required.
· Must obtain Nonviolent Crisis Intervention within 60 days of hire.
This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).

This is a Full time day shift position without weekend requirements.
