Job Summary and Qualifications

  • Creates and maintains a process for managing and tracking acute allogeneic transplant patients
  • Provides a single point of contact for referring physicians and acute allogeneic patients in outreach areas
  • Provides a single point of contact for allogeneic patients during the acute phase of transplant in the outpatient setting
  • Partners with physicians and mid-level providers to track and manage immunosuppressive drug levels and dose changes
  • Tracks and manages CMV/EBV levels and collaborates with physicians and mid-level providers to initiate therapy where appropriate
  • Meets with patients regularly during clinic visits for focused nursing assessments and communicates with provider team
  • Collaborates with the nursing team r/t complications or side effects and recommended actions
  • Partners with the nursing team to schedule and coordinate supportive therapies and appointments
  • Coordinates care using the multi-disciplinary team resources
  • Communicates with inpatient and outpatient care teams to ensure continuity of care for acute allogeneic patients
  • Ensures accuracy and completion of the acute GVHD grading worksheets

What qualifications you will need:
Licensure/Certification/Registration:
  • Current licensure in the State of Colorado as a Registered Nurse
  • BLS Certification through The American Heart Association
  • ACLS required within 9 months
  • NIH Stroke Scale certification required within 3 months of hire
  • ONS Chemotherapy Provider Card required within the first 9 months of hire. If the staff member is prn, they will not be required to have ONS Chemotherapy Provider Card because they will not have enough exposure to chemotherapy/biotherapy to maintain competency in this area.
Education/Experience/Special Qualifications:
  • Graduate of an accredited school of nursing
  • EKG interpretation competency or course within the first year of hire. BSN preferred.
  • One year experience in acute care preferred.
  • Ability to communicate effectively with patients, families, physicians, and other health care team members. Ability to manage a team and work with others within a team to ensure quality patient care. Strong critical thinking skills. Desire to provide compassionate, holistic care.

Position Summary:

  • Creates and maintains a process for managing and tracking acute allogeneic transplant patients

  • Provides a single point of contact for allogeneic patients during the acute phase of transplant in the outpatient setting
