Job Summary and Qualifications

Seeking a Cardiovascular Services Care Coordinator to join our team who is a professional nurse who is responsible for the overall coordination of the Phase I cardiac rehabilitation program. This includes performing patient teaching and nursing interventions to enhance strengths and minimize weaknesses of patient, families, and nursing staff involved in the Cardiac Rehabilitation Program. Utilizing an interdisciplinary approach, the Cardiovascular Care Coordinator will collaborate with the nurses and physicians involved in the care of patients with cardiac diagnoses.

What You Will Do In This Role:

  • You will plan, coordinate, implement, document, and evaluate educational, exercise and behavioral counseling components of the Cardiac Rehabilitation Program. Assess teaching/learning needs of patients and families regarding cardiac and vascular rehabilitation.
  • As the Cardiovascular Care Coordinator you will serves as a resource or consultant to nursing units caring for patients with cardiovascular disease, to the Education Department, and to other hospital personnel.
  • You will consult with nursing units caring for patients with cardiovascular disease, Education Dept., and other hospital personnel/units to provide and implement plans of care
  • You will maintain a quality assurance program for cardiac rehabilitation, to include the development/update of policies & procedures related to cardiac rehabilitation, prepares statistical reports and identifies areas for improvement. Serves on committees regarding quality of care issues to help determine appropriate action plans, which relate to Cardiac Rehab.
  • You will keep abreast of the physician utilization of service, their specialty, and specific needs. Coordinates this information with departments and their supervisors to ensure smooth transition of the patient through the Cardiac Rehabilitation program.

What Qualifications You Will Need:

  • Graduate of an accredited School of Professional Nursing
  • Baccalaureate Degree in nursing preferred.
  • Certification in BLS, ACLS, Basic Dysrhythmia Course.
  • License to practice professional nursing in the State of Texas
  • Critical care experience with experience in cardiovascular assessment.
  • Knowledge of and ability to apply principles of adult education.
  • Supervisory experience is recommended but not required.
