Job Summary and Qualifications

As an RN Case Manager for HCA , you will be joining a team of professionals who are dedicated to ensuring our patients receive excellent care, both in hospital and after discharge. You will enable patients to reach their goals through care and actions that are crafted specifically to individual patient needs and administered by some of the most highly skilled and trained providers. What you will do in this role: 

• You will be responsible for the coordination of services through the continuum of care that balances the delivery of quality patient care with the cost effective utilization of resources. 
• You will perform concurrent clinical review to determine appropriateness of admission and continued need for care delivery in the acute hospital setting. 
• You will be supporting, collaborating and communicating among the patients, families and entire health care team regarding the patient's plan of care and continued needs post-discharge and facilities those services may require. 
• You will be performing telephonic clinical reviews with claims insurance personnel to facilitate reimbursement for the delivery of patient care services. 
• You will identify quality and or risk issues and report those to the appropriate individual and or department. 

What qualifications you will need:

• Current Licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-State Licensure as a Registered Nurse
• Current Basic Life Support (BLS) verification course through AHA or ARC 
• Bachelor's prepared RN with current state licensure. In lieu of BSN, RN must have 5 years case management experience and complete a bachelor’s of science in nursing within 2 years of hire date. 
• 5 years of nursing experience with 2 years critical care or 3 years nursing experience 
• 2 years case management experience preferred 
• Recent hospital social work or case management experience preferred
