Job Summary and Qualifications

  • Accurately collects and documents admission and shift observation and clinical data. Provides appropriate information to assist RN with admission and shift RN assessments.
  • Performs vital signs, BP's and accuchecks. Provides or assists patients with Activities of Daily Living. Assists with treatments or procedures within documented level of competency.
  • Provides patient care (including medication administration) and related activities, services, and/or procedures to promote the physical and emotional health and personal comfort needs of patient. Provides and documents patient/family education.
  • Provides behavioral health care interventions utilizing the least restrictive interventions, and consistent with a culture of recovery and Trauma Informed Care.
  • Provides for the safety of patients, visitors and staff. Completes routine and special safety checks.
  • Assumes responsibility for completing all mandatory educational, PPD, CPR and CPI requirements within designated time frame and for providing written verification of same for filing.
  • Actively participates in quality management activities by timely reporting of staff, family, visitor or quality concerns.
  • Assumes responsibility for reviewing all work related communications and takes the initiative to seek clarification when needed.
  • Facilitates and/or assists with patient community, education or activity groups. Documents patient's participation.
  • Assists with clerical duties as assigned.
  • Demonstrates knowledge of, and follows policy/procedure, for all hospital patient safety initiatives.
  • Provides safe Transport for patients to and from Behavioral Health Services for any necessary diagnostic test that may be required for their care.

What qualifications you will need:

  • Graduate of a Practical Nursing Program- Required
  • Licensed Practical / Vocational Nurse License (LPN / LVN)-Required
  • Basic Cardiac Life Support-Required
  • Nonviolent Crisis Intervention must be obtained within 60 days of hire or start in position- Required
  • A Bachelor’s degree or college course in health care, mental health, recreational therapy, education, or human growth & development-Preferred
