Job Summary and Qualifications

We are seeking a Social Worker Case Manager for our team to ensure that we continue to provide all patients with high quality, efficient care. Did you get into our industry for these reasons? What you will do in this role:

As a Social Worker Case Manager, you assist our patients in finding the services they need for continuity of care upon leaving the hospital. Our team collaborates closely with our community partners to provide services such as home health, hospice, skilled nursing, assisted living, private care, and durable medical equipment (DME). 

• You will manage the patients’ care from admission to discharge.
• You will collaborate with the health care team and develop treatment goals and plan interventions that assist the patient and family to resolve psychosocial concerns that may arise as a result of illnesses, limited finances and/or physical disabilities.
• You will document patients’ social work assessments, treatment goals, interventions, outcomes, and discharge plans. 

What qualifications you will need:

• Graduate of an accredited school of social work.
• Bachelor degree in Social Work or related Human Services field.
• MSW preferred.
• Licensed Clinical Social Worker preferred.
• 2 or more years of social work experience in an acute care setting preferred.
• Previous case management or utilization review experience preferred.
