The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.
EDUCATION REQUIRED AND/OR PREFERRED:
Associate Degree preferred.
EXPERIENCE REQUIRED OR PREFERRED:
Minimum of one year experience in a health care setting required.
SKILLS, KNOWLEDGE, AND ABILITIES:
Medical knowledge and terminology required. Demonstrated superior communications and customer service skills, success in organizing work, computer and typing skills, and ability to multi-task. Ability to establish and maintain collaborative and effective working relationships. Ability to communicate effectively in oral, written, and electronic formats. Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills. Demonstrates an ability to perform specific competencies as identified. Ability to be self-motivated, goal-directed, and dependable.
Current FL license in CNA or EMT preferred.
This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
