Job Summary and Qualifications

The Medication History Pharmacy Technician specialist performs medication histories for new hospital admission. We are amazing team that works hard to support each other and are seeking a remarkable addition who feels patient care is as meaningful as we do.

What you will do in this role:

• Interview patients and family members for the purpose of compiling an accurate and complete list of current medications the patient was taking prior to admission to the hospital, to include all prescription, non-prescription, and investigational medications.
• Contacts the patient’s community pharmacy, physician’s office, or other healthcare facility for clarifications as needed.
• Accurately updates medication history, allergies, adverse drug reaction, pharmacy, and primary care provider information in the electronic health record.
• Correctly and completely documents interventions made to the medication history in the electronic medical record prior to the end of each shift worked.
• Discuss any questions or issues that arise during the medication history process with a clinical pharmacy specialist.
• Appropriately communicates unresolved medications history issues to clinical pharmacy specialist.
• Documents patient medication history in the electronic health record and CPW/VigiLanz.
• Ascertains need for language assistance and obtains interpreter as necessary.
• Assists the clinical pharmacist performing education and/or discharge counseling through identification of patients that need medication histories complete by pharmacy services and preparation of counseling materials.
• Perform other duties as assigned.

What qualifications you will need:

• A high school diploma or equivalent is required
• An active Registered Pharmacy Technician license in the state which you practice, in accordance with law and regulation is required
• A national certification (CPhT) is required
• 2 years of experience as a Pharmacy Tech required
