Job Summary and Qualifications

As a Case Manager, you assist our patients in finding the services they need for continuity of care upon leaving the hospital. Our team collaborates closely with our community partners to provide services such as home health, hospice, skilled nursing, assisted living, private care, and durable medical equipment (DME). 

What you will do in this role:

• You will manage the patients’ care from admission to discharge.
• You will collaborate with the health care team and develop treatment goals and plan interventions that assist the patient and family to resolve psychosocial concerns that may arise as a result of illnesses, limited finances and/or physical disabilities.
• You will document patients’ social work assessments, treatment goals, interventions, outcomes, and discharge plans. 

What qualifications you will need:

• Graduate of an accredited school of nursing or accredited school of social work.
• BSN or MSW preferred.
• Licensed Registered Nurse.
• Previous case management or utilization review experience preferred.
